Recently moved to Illinois and I am seeking position as MDS nurse, Case management or utilization review. I have 20 plus years of experience in Ohio as a MDS Coordinator and case manager. I have worked in single facilities as small as 50 beds and multiple facilities at one time as large as 145 beds. I have handled a case load of 140 beds with 20-30 skilled performing insurance updates, completion of the MDS and care plan review. Attended meetings and care conferences as will at weekly team meetings for skilled resident. I have an active Illinois and Ohio nursing licenses and can work remote or on site.
