As a Patient Health Navigator and supervisor, my responsibilities are to oversee and supervise five other Patient Health Navigators in our Federal Qualified Health Center clinic sites. Implementing innovative strategies for enhancing overall efficiency in managing caseloads while maintaining high standards of patient care. Having weekly PHN zoom meetings with them to update them on any new changes with health coverage at state and federal level, providing educational information or added tasks. Making sure we are all working as a team with the same goals in mind. To make sure our organization is in compliant with certain grants I will submit weekly PHN reports to the Illinois Primary Health Association and the Get Covered IL organizations. Participate in monthly Leadership meetings where I present a PowerPoint with PHN's goals, objectives and measure updates. As a certified application counselor, my main job is to help clients apply for benefits such as, IL Medicaid, SNAP benefits, Marketplace insurance, Charity Care Programs and IBCCP applications. My goal is to identify the the clients situation and educate them on different benefits that they would benefit from depending on their financial situation. After helping them with their applications I follow up on their case with the caseworker at their local DHS office either by emails or phone calls. I will let client know of any missing information and guide them on their next steps. Making sure they understand and have clarification on the process.
Processing our Mendota's clinic site's vouchers and bills. These include vouchers from Mendota specialist and Fox Valley Imaging vouchers. This will consist of gathering of the returned vouchers from the providers, HICFA bills and Dr. office notes for date of service. I make sure this is all complete for each voucher. I will enter these into the provider voucher excel sheet and put in a PO for payment.
I work with all dental referrals from the Mendota clinic patients. I will call them to make sure they have found a specialist that takes their insurance plan and have an appointment.
I 'm also helping with the scheduling of the Well women visits after their IBCCP applications have been approved. Once the patient has been seen and billed for I will sent the proper documentation to the LaSalle County for payment on these applications.
Scrubbing medical schedules a day before. Making sure the correct insurance has been entered and they are eligible for services. Using the Medi to verify state insurance and Provider portals for commerical Insurance. Making sure the correct MCO has been entered and changed when needed. Also confirming when a child is coming in for well child and immunizations visit if they eligible to receive our VFC immunizations. Putting in pop ups in the registration if they are not and letting the nurse as well.
Participate in outreaches by helping fill out the registrations and HRA forms, entering of the data and providing information on CHP services and PHN services.

I will enter these into the provider voucher excel sheet with a check requisition and send to Administration office for the payment of bills. This will be done no later than the 15th of each month.
I am also responsible for the Mendota Quest Bill. I will receive the Quest bill each month with a list of patients who have had labs the month before. I will make sure these are all sliding fee patients and the labs are billed to CHP correctly. I will sent this to the administration office as well for payment.

As a front office assistant I greeted patients coming into the clinic. Checking them in for their medical or dental visit. Filling out a full family registration for their visit. Verifying their family income and obtaining profile and consents. Scheduling of their medical and dental visits in person and over the phone. Checking out the patient and collecting the charges for the visit or submitting it the their insurance. Verifying patient's insurance coverage, deductible and co-pay. Working the panel roster and verifying what a child is in need of. Whether it is a well child visit, lead screening, vision screening or development screening. Billing for the voucher system. Making sure all voucher, bills and Dr. Notes have been collected. Managing of the medical and Dental appointment schedules. Handling of phone calls and forwarding them to the designated person. Communication any messages from patients to staff and vice versa. Balancing at the end of the daily, making sure all credit card, cash and checks were entered properly. Doing the daily cash sheets, scanning and filling them correctly.
As an ESL Aide my job was to work with kindergarten and first grade students with little or no english comprehension. On a daily basis I would either work with them one on one or in a small group. I was responsible for preparing my own group lesson and providing the tools needed for them. When I wasn't in a group I was with one of the teachers in their class room. I was needed to observe and ensure the students were working on target. Helping when they seem confused with class room discussion due to the language. Repeating teacher's instructions or questions for the students. Not only was I needed by the students or teachers, but also for the parents. I did a lot of translating between teacher an parents to help with the language barrier. Making sure parent's concerns or questions were related to the parent. I also participated in parent and teacher conferences for the success of the students. When ever there was a letter or note that needed to be sent home to one or all of the students I needed to translate it in Spanish.
Youth Service provided an ESL class for adults and their families. I would provide child care for their young children while the parents where in class. If older I would help them with their homework or give them activities to work on. Always focusing on reading and writing.
Working at Plano Molding my job consisted of working the assembly line. Working with plastic mold assembling them into tackle and caboodle boxes. Looking for any imperfections, making sure correct bar codes and labels where placed. Boxing them and sending to the dock department for shipping out.
I worked with the librarian on staff by helping with the checking out and in of books. Filing books on the shelves, helping with the summer programs, filling out registrations for library cards, documenting of new books coming in and old books being discontinued from the library, helping people order books needed from other libraries to check out, and helping Illinois Valley Community College to take their tests on the system for their online classes.
I have my Certified Application Counselor license since 2017 and is current.
