Program that helps patients navigate the health system, my duties:

▪ Inpatient Program: served readmitted and high-risk patients. Performed in person visits at bedside/site of care to provide navigation of the health care system for them

▪ Performed morning and afternoon rounding with the Oncology, Gastroenterology and Hospitalists Physicians teams.

▪ Outpatient Program: served all patients referred for follow-up care. Phone outreach by TSP Navigator and longitudinal follow-up.

▪ Participated in multiple simulation lab training for bedside skills education. ▪ Organized and hosted Future Health Leaders Advocate Health Care events: committed to educating motivated high school and college students interested in the health care field through presentations and simulation labs.

• Assisted radiologists in performing diagnostic imaging procedures, such as x-rays and ultrasounds.
• Maintained patient records and updated information in the radiology database.
• Prepared and positioned patients for imaging exams according to established protocols.
• Reviewed images for accuracy prior to submitting them for interpretation by the physician.
• Educated patients regarding preparation for radiology exams and post-procedure care instructions.
