1. Provides crisis stabilization services off-site in clients natural environments following mobile response services.

2. Facilitates communication externally and across agency staff applicable programs to ensure continuity of care

3. Provides timely phone and in person follow up with individuals, families. and inpatient systems.

4. Serves as the point of contact between Arrowleaf and inpatient facilities to exchange information and develop a plan of acre following discharge including, but not limited to counseling and psychiatric linkage.

5. Provides case management services, onsite and offsite as identified by clients needs.

6. Conducts outreach to individuals who are at risk of entering crisis systems of care.

7. Operates as the initial access point for individuals calling Arrowleafs 24 hours recourse and crisis line.

8. carries current CPR. cert.

1. Maintains a valid drivers license and insurance.

2. Maintain First aid and CPR. certification

3. Maintain a current Illinois CNA. certification

4. Implements residents specific goals as outlined in the residents service care plan.

5. Completing routine health checks using basic in-home health care equipment.

6. Reminding residents of their medications and treatments.

7. Assists in meal prep. meal delivery and clean up.

8. Completing all other cna tasks required by Arrowleaf.
